- Address, city/state/ZIP, telephone.
- Account number(s).
- TIN.
- Authorized signer printed names.
- Titles.
- Manual or facsimile signature indication.
- Signature boxes.
- Business entity certification.
- Authorized signature/title and date.
- Internal verification fields.
Key Instruction
The form instructs signers to use black or blue ink and keep signatures within box boundaries. It also tells depositors to line out unused signature boxes before signing.
