Purpose
This document is intended to confirm the principal party or authorized principal relationship for a transaction or service request.
Reconstructed Source Outline
The template is titled Confirmation of Principal Contract / Affidavit / MFPA.
It is used when an intermediary, broker, mandate, or representative introduces a principal party to US Capital Private Bank and asserts that they are acting on behalf of the principal for a transaction.
The document includes:
- Transaction code.
- MFPA transaction code.
- Introductory language acknowledging receipt of an offer of services.
- Language referencing bank secrecy, privacy, and limitations on disclosure to intermediaries or brokers.

- A note that the principal signature agrees to engage directly in transaction/sale/service discussions and indicates best times to call.
The template states that private information cannot be disclosed to brokers/intermediaries/mandates/representatives without direct permission from the principal party in interest. Once the introduction is made, further communication must be made principal to principal or bank to bank.
