The VOD request form is used to request direct verification of deposit/account information from a depository institution. It is intended to move directly between the requester/lender and the depository, not through the applicant or other interested party.

Form Summary

The form includes:

  • Requesting officer/lender information.
  • Applicant authorization.
  • Account information to be verified.
  • Depository-completed section for account type, account number, current balance, average balance, and date opened.
  • Loan/outstanding obligation fields.
  • Additional information that may assist creditworthiness review.
  • Authorized depository representative signature, title, date, phone, and email.

Client-Safe Reply

If a verification of deposit is required, please complete the applicant authorization section and follow the instructions provided by the team. The verification should be returned directly by the depository institution to the requester, not routed through the applicant.
